Maybe it’s just me with my medical knowledge. Uterine tear is a rupture and requires delivery of the baby. Unless in this fictional world they have some new surgery technique that allows the to repair the rupture and continue the pregnancy. Rupture tends to result in a hysterectomy.
Dom should go to sleep and find Ella in dream to wake her up unless she is in limbo talking with her mom.
