The intensive care waiting room of the Salvator Mundi hospital in Rome smelled entirely too sterile. It lacked the comforting, heavy scent of old books and lavender that Lily always carried.
I sat in a hard plastic chair, staring blankly at the double doors leading into the surgical wing. My combat shirt was stiff with dried blood some of it mine, most of it Lily’s. I hadn’t let the nurses examine my ribs. I couldn't focus on my own pain when my sister was currently having her abdomen stitched back together.
Cassian sat beside me, his arm wrapped firmly around my shoulders. He was an immovable, solid anchor in the middle of a hurricane.
Leo was asleep on the vinyl sofa across from us, his head resting on Matteo’s heavy tactical jacket. The Capo stood guard by the door, an assault rifle resting casually against his hip, his eyes sweeping every nurse and doctor who dared to walk past the waiting room.
"She’s strong," Cassian murmured, his thumb gently tracing the line of my collarbone. "She survived three years in a coma. A laceration won't keep her down."
"If she dies," I whispered, my voice completely devoid of emotion, "I am going to burn Europe to the bedrock."
Cassian didn't tell me I was overreacting. He didn't offer a platitude. He simply pressed a kiss to the top of my head and said, "I'll light the match."
The double doors swung open.
A tired, silver-haired surgeon stepped into the waiting room, pulling down his surgical mask. He recognized Cassian immediately, offering a brief, respectful nod to the Don before looking at me.
"Donna Rossi," the surgeon began, his voice calm. "The blade missed the descending aorta by two millimeters. She lost a significant amount of blood, and we had to repair the intestinal wall, but the surgery was successful."
My lungs, which had been locked in a state of suffocating tension for three hours, finally released a ragged, shaking breath.
"She is stable," the surgeon continued, offering a reassuring smile. "She is resting in a secure recovery suite. We expect a full recovery."
I stood up, my legs trembling slightly. "I need to see her."
"Of course," the doctor nodded. "But only family."
Cassian stood up with me, his hand resting on the small of my back. He looked at Matteo.
"Stay with Leo," Cassian ordered quietly. "Nobody enters this floor without my explicit permission. If a stranger steps off that elevator, shoot them."
"Yes, Boss," Matteo nodded, shifting his stance to fully block the corridor.
Cassian and I followed the surgeon down the quiet, sterile hallway. He led us into a large, private recovery suite overlooking the glittering night skyline of Rome.
Lily lay in the center of the bed, connected to a labyrinth of IV tubes and heart monitors. She looked incredibly pale, but the steady, rhythmic beep of the electrocardiogram was the most beautiful sound I had ever heard.
I walked to the side of the bed, gently picking up her limp hand, being careful not to disturb the IV line. I pressed my lips to her knuckles, closing my eyes.
"I'm here, baby bird," I whispered.
Lily didn't wake, but her breathing seemed to deepen, comforted by the familiar presence.
Cassian stood on the opposite side of the bed, looking down at his sister-in-law. His jaw was locked tight, his dark eyes brooding with a dangerous, calculating intensity. The relief was there, but it was overshadowed by the looming specter of the war we had just been thrust into.
Cassian pulled the encrypted satellite phone from his pocket.
He didn't dial a number. He simply turned the device on and placed it on the small table next to Lily's bed.
"The mercenary in the cellar told me this was a coordinated strike," Cassian said softly, not looking away from Lily. "He said the attack on the villa was a distraction."
I looked up at him, my brow furrowing. "A distraction for what?"
Before Cassian could answer, the satellite phone vibrated.
It wasn't a standard ringtone. It was a single, sharp chirp. The encrypted frequency we hadn't used in ten years.
I let go of Lily's hand and picked up the phone. I pressed the answer button.
"Speak," I commanded.
Static hissed over the line, followed by the heavy, rhythmic sound of rain hitting a tin roof.
"Master," the flat, hypnotic voice of Viper Nine echoed into the quiet hospital room.
My heart skipped a beat. Nine was supposed to be bleeding out in the ruined chapel in Tuscany. We had left him with Luca’s medical team, but his injuries had been catastrophic.
"Nine," I breathed, profound relief mixing with sudden, terrifying confusion. "You're alive."
"The Rossi medics stabilized me," Nine reported, his voice weak, strained with obvious pain, but undeniably alive. "But I am not calling to report my status."
"Where are you?" Cassian asked, stepping closer to the phone.
"I am currently in transit to a secure location in Geneva," Nine replied. "But the network is awake, Master. The beacon was triggered."
"What beacon?" I demanded.
"When the Praetorians attacked me in the chapel," Nine explained slowly, each word costing him effort, "I didn't just broadcast a localized SOS to the Rossi servers. I initiated the Blackfall Protocol."
The blood completely drained from my face.
The Blackfall Protocol. Arthur had designed it as a fail-safe in the event the Guild's leadership was ever compromised. It was an encrypted, global broadcast sent to every sleeper agent, every dormant assassin, and every ghost the Weaver had ever trained.
"You woke them up," I whispered, the sheer magnitude of the tactical shift hitting me. "All of them."
"Forty-nine ghosts," Nine confirmed, a dark, fanatic reverence bleeding back into his tone. "They lived in the light for ten years, Elara. But they never forgot the dark. And they never forgot who set them free."
"Nine, what is happening?" Cassian demanded, his voice hardening into the authoritative growl of the Don.
"The High Table remnants attempted a synchronized, global decapitation strike tonight," Nine reported. "They sent kill squads to the homes of Don Salvatore, Don Moretti, and the remaining European syndicate heads. They intended to wipe out the Five Families while you were distracted in Tuscany."
I looked at Cassian. The geopolitical nightmare was unfolding exactly as the Emissary had predicted a decade ago.
"Are the Dons dead?" Cassian asked, his grip tightening on the edge of Lily’s bed.
A low, chilling laugh echoed through the satellite phone. It was the sound of a predator realizing it had finally cornered its prey.
"The kill squads never reached the front doors," Nine whispered. "The ghosts intercepted them. Every single High Table assassin dispatched tonight was executed before they could pull a trigger. The Five Families are completely secure."
Cassian and I stared at the phone in absolute, stunned silence.
Fifty orphans. Fifty kids I had paid to walk away and live normal lives had just orchestrated the most flawless, coordinated counter-offensive in the history of the underworld. They hadn't fought for money. They hadn't fought for power.
They fought because their Master’s family was threatened.

I looked up at Cassian.

I slowly raised the phone back to my ear. The terrified mother was gone. The Donna of the Rossi Syndicate was gone.

"Nine," I spoke into the receiver, my voice cold, precise, and echoing with the promise of absolute ruin. "Rest and recover. When you wake up, tell the network to initiate a global sweep. I want the names and locations of every High Table loyalist left on this earth."
I looked out the hospital window, staring at the glittering, eternal city of Rome.
