The stench of formalin and scorched copper was the first thing that hit Caelen Voss.

The second was the blinding, rhythmic pulse of crimson light directly above his eyes.

"Dr. Voss? Are you listening, or has your mind completely rotted away like your reputation?"

Caelen snapped his head up. He wasn’t in the sterile, air-conditioned operating theater of St. Jude’s Memorial. He wasn’t wearing his surgical scrubs.

He was standing behind a dark oak lectern in a massive, cold-stone amphitheater. Below him sat roughly two hundred young men and women in high-collared, navy-blue academy uniforms, all staring up at him with expressions ranging from sneering contempt to outright boredom.

Where am I? Caelen thought, his lungs tightening.

A sharp, searing pain shot through his temples, dragging a flood of alien memories into his mind.

He was Caelen Voss. But not just the disgraced trauma surgeon from Earth who had been framed by corrupt hospital executives for exposing an organ-harvesting ring. He was also Caelen Voss—a penniless, disgraced forensics instructor at the Chrono-Medical Academy in a world called Chrono-Earth.

And then he saw the reflection in the dark glass window to his left.

Floating two inches above his forehead, written in burning, neon-red digital glyphs, was a ticking countdown:

[ 00 : 23 : 59 : 52 ]

[ 00 : 23 : 59 : 51 ]

[ 00 : 23 : 59 : 50 ]

Twenty-three hours. Fifty-nine minutes. Fifty seconds.

What the hell is this?

Before he could process the numbers, the heavy double doors at the top of the amphitheater slammed open with a metallic crash.

" clear the hall!" a harsh voice boomed across the auditorium.

Six guards clad in silver-and-black ceremonial plate armor marched down the stone steps, their hand-held spearheads glowing with a pale blue energy. Behind them walked a man in his late thirties, wearing a immaculate white doctor’s coat trimmed with gold thread. His dark hair was slicked back, and his face was twisted into a smug, triumphant grin.

Dr. Valerius Sterling. Chief Medical Director of the Upper Academy.

The students instantly stood up, bowing their heads in deference.

"Director Sterling!" several students whispered in hushed awe.

"What is he doing in the Lower Precinct?"

Valerius paid no attention to the students. His polished leather boots clicked rhythmically against the stone stairs as he descended toward the podium, stopping just ten feet from Caelen.

Above Valerius's head floated a thick, shimmering golden counter:

[ 142 Years : 08 Months : 12 Days ]

"Caelen Voss," Valerius purred, leaning his hands against the lectern. "The Board of Directors has finalized its decision. Your academic license is revoked, effective immediately. Your remaining Life-Span Currency is confiscated to pay off your mounting institutional debts."

Caelen’s eyes narrowed. Life-Span Currency. The memories clicked into place with horrifying clarity. In this shattered alternate Earth, physical money didn't exist. Time was the sole currency. If your counter hit zero, your body withered into dust within seconds.

"My debt?" Caelen’s voice was raspy, but his tone remained dead calm. "I don't owe the Academy a single hour."

"You do now," Valerius sneered, pulling a gold-embossed scroll from his coat. "Malpractice, unauthorized research, and gross incompetence. The Board has ordered an immediate liquidation of your remaining hours. Guards, seize him and connect the Siphon-Lances."

The guards stepped forward, raising their energy-tipped spears.

"Wait!"

A high-pitched gasp echoed from the front row. A young male student—wearing a tailored silk uniform bearing the crest of the House of Vane—suddenly clutched his throat. His eyes rolled into the back of his head, his face turning a deep, unnatural shade of violet. He collapsed forward, smashing his face against the wooden desk before sliding onto the stone aisle, convulsing violently.

The hall erupted into panic.

"Young Master Julian!" one of the female students shrieked. "He’s suffocating!"

Valerius raised a hand, waving his guards back. "Silence! Everyone remain calm!"

Valerius stepped toward the fallen student, kneeling beside him with a practiced, elegant posture. He pressed two fingers against Julian’s neck, barely glancing at the boy’s chest, before rising back up with a solemn, sorrowful sigh.

"Chrono-Fatigue Syndrome," Valerius declared loudly, his voice echoing off the stone walls. "A sudden, fatal collapse of the life-force pathways. His body is rejecting its remaining years. There is no cure."

Above the boy’s head, a pale blue counter read: [ 18 Years : 04 Months : 02 Days ]. But the numbers were flickering wildly, dropping by months every second.

"What a tragedy," Valerius continued, a cold, predatory glint flashing in his eyes. "To prevent the waste of such precious Life-Span Currency in a dead vessel, as Chief Director, I authorize an emergency Mercy Extraction. Guards, attach the siphons to Young Master Julian. Transfer his remaining eighteen years to the Academy’s reserves."

The students gasped, but no one dared speak out. In this world, the diagnosis of a Chief Director was absolute law.

The lead guard lowered his siphon spear, pointing the needle-sharp tip toward the unconscious boy’s neck.

Ping!

A sharp, chime echoed directly inside Caelen’s brain, freezing time for a fraction of a millisecond.

[ SYSTEM INITIALIZATION: DEATH-SEEKER'S DIAGNOSTIC EYE ACTIVATED ]

[ TARGET DETECTED: Julian Vane ]

[ OFFICIAL DIAGNOSIS: Chrono-Fatigue Syndrome (INCORRECT / FRAUDULENT) ]

[ ACTUAL DIAGNOSIS: Acute Mechanical Airway Obstruction via Foreign Object & Secondary Neurotoxin Injection ]

[ CAUSE OF IMPENDING DEATH: Asphyxiation induced by corrupt medical sabotage ]

[ TIME TO DEATH: 00 : 02 : 14 ]

[ PERPETRATOR: Dr. Valerius Sterling (Assistant planted toxin 10 minutes prior) ]

Caelen’s eyes widened. A grid of glowing, translucent lines overlaid his vision, highlighting Julian’s neck in bright neon yellow. Inside the boy’s trachea, Caelen could clearly see a swollen, inflamed obstruction blocking air from entering his lungs, accompanied by a dark purple venom slowly creeping down his carotid artery.

It wasn't a natural collapse. It was a manufactured murder—a blatant attempt by Valerius to harvest eighteen years of noble blood under the guise of an "emergency mercy extraction."

The guard raised the siphon lance, inches from the boy's jugular.

"Stop right there, you incompetent butcher!"

Caelen’s voice thundered through the hall, sharp enough to cut through the panic like a scalpel.

The entire room went dead silent. The guard froze mid-strike.

Valerius turned around slowly, his eyebrows twitching in disbelief. "What did you just say to me, Caelen?"

"I said you’re a hack," Caelen said, vaulting over the wooden podium with practiced ease and landing heavily on the stone floor. He strode toward Valerius, his dark coat billowing behind him. "Chrono-Fatigue? Are they handing out Chief Director titles to blind fools nowadays?"

"You dare insult my diagnosis?" Valerius’s face flushed red with rage. "I am a Senior Master of the Medical Guild! You are a disgraced, penniless instructor with less than a day to live!"

"Your title doesn't change the anatomy," Caelen snapped, stepping directly into Valerius’s personal space. "The boy isn't suffering from Chrono-Fatigue. He’s suffocating because a foreign object is jammed in his subglottic airway, paired with a localized paralytic toxin that you—or your lapdogs—injected into his morning tea."

The students gasped. Several noble heirs in the front rows began whispering furiously among themselves.

"Lies! Slander!" Valerius roared, pointing a trembling finger at Caelen. "Guards! Execute this lunatic immediately! Siphon every last second from his head!"

Two guards lunged forward with their spears.

Caelen didn't even look at them. His kinetic vision accelerated his perception. He sidestepped the first spear thrust with effortless precision, grabbed the guard’s wrist, and twisted it downward. The weapon clattered to the stone floor.

Before the second guard could react, Caelen reached into his coat pocket, snatched a heavy silver fountain pen from his notebook, and slammed the second guard’s helmet visor upward with his elbow.

"Touch me again, and I’ll sever your brachial artery before you can pull that trigger," Caelen growled, his voice dropping to a terrifying, frigid whisper.

The guards hesitated, terrified by the sheer, unyielding dominance radiating from a man who was supposed to be a helpless scholar.

Caelen ignored them, dropping to one knee beside the gasping boy.

[ COUNTDOWN: 00 : 01 : 12 ]

[ CAELEN VOSS EXPIRATION: 00 : 23 : 48 ]

"He’s dying!" a student cried out. "His skin is turning black!"

"He has one minute," Caelen said calmly. He ripped open Julian’s high-collared silk shirt, exposing the boy’s pale throat. "I need a blade and a hollow tube. Now."

"Do not touch him!" Valerius screamed, drawing a short, glowing scalpels-blade from his belt. "If you touch a noble heir without authorization, it is high treason!"

"Shut up and watch how a real doctor works," Caelen snapped without looking up.

With a swift motion, Caelen unscrewed the metal cap of his silver fountain pen, tossing the ink cartridge aside until he was left with a hollow, rigid silver tube.

He placed two fingers on Julian’s neck, feeling for the cricothyroid membrane—the tiny space between the thyroid and cricoid cartilages.

"No surgical blade?" Caelen muttered under his breath. "Fine."

He picked up the sharp, broken metal clip from the pen’s cap.

The entire hall held its breath. Even Valerius stood frozen, his eyes wide with horror and shock as he realized what Caelen was about to attempt.

"Incision," Caelen whispered.

With terrifying speed and surgical precision, Caelen sliced a small, half-inch horizontal cut directly through the boy’s skin and membrane. Blood welled up, but Caelen didn't flinch. He inserted his gloved pinky finger into the incision, opening the airway, and shoved the hollow silver fountain pen directly into the hole.

Psssshhhh!

A sharp, violent rush of trapped air exploded out of the silver tube.

Julian’s chest suddenly heaved upward. The boy gasped violently, pulling a deep, ragged breath of oxygen directly through the metal pen stuck in his throat. The dark purple tint on his face immediately began to fade, replaced by a flush of natural red.

[ EMERGENCY CRICOTHYROIDOTOMY SUCCESSFUL ]

[ AIRWAY RESTORED ]

The room erupted into wild, chaotic murmurs.

"He... he bled him through the neck!"

"Look at his breathing! His chest is moving!"

"He saved him with a pen?!"

Caelen stood up slowly, wiping a spot of blood from his cheek with the back of his hand. He looked down at the boy, whose blue life-span counter had stopped wildly flickering and stabilized at a steady [ 18 Years : 04 Months : 02 Days ].

Then, Caelen turned his gaze back to Valerius.

"Now," Caelen said, his eyes burning with a cold, predatory light. "Let's talk about that poison you planted."

[ SYSTEM NOTIFICATION ]

[ SYSTEMIC FRAUD & MEDICAL MALPRACTICE DETECTED ]

[ VERDICT: CHIEF DIRECTOR VALERIUS STERLING GUILTY OF ATTEMPTED SYSTEMIC MURDER ]

Above Valerius's head, the brilliant golden numbers began to shake violently.

Valerius grabbed his own throat, his eyes bulging in sheer terror as he looked up at the ceiling. "No... NO! WHAT IS THIS?!"

A violent stream of golden light ruptured from Valerius’s forehead, shooting straight up into the air before curving downward like a lightning bolt—and slamming directly into Caelen Voss’s chest.

[ PENALTY ENFORCED: 10 YEARS CONFISCATED FROM PERPETRATOR ]

[ ABSORBING LIFE-SPAN CURRENCY... ]

Caelen felt a rush of raw, unadulterated energy surge through his veins. The fatigue in his muscles vanished. His mind felt sharper than it had in a decade.

Above Caelen’s head, the burning red digits shattered, melting away into a brilliant, dazzling gold:

[ 10 Years : 00 Months : 23 Hours : 42 Minutes ]

Meanwhile, Valerius collapsed to his knees with a horrific shriek. His sleek dark hair turned silver-white in seconds. Deep, heavy wrinkles carved themselves into his cheeks, and his posture hunched over as if weighed down by decades of sudden age.

Above Valerius’s head, his proud counter shrank from 142 years down to [ 132 Years : 08 Months : 12 Days ].

The entire lecture hall went deathly silent. No one moved. No one dared to breathe.

Caelen stepped over the groaning, aged Director, reaching down to casually pick up his leather bag from the podium. He adjusted his collar, looking down at Valerius with supreme contempt.

"Class dismissed," Caelen said quietly.

He turned and walked toward the exit. But just as his hand touched the heavy brass handle of the doors, his Diagnostic Eye flashed again, highlighting a thick, black paper envelope tucked quietly into the inner pocket of his coat.

Caelen pulled it out. Written on the heavy cardstock in blood-red ink was a single line:

“You survived your execution, Doctor. But you have 24 hours before every student in this academy bleeds out. - The Poison Guild.”

Caelen’s lips curled into a dangerous, razor-sharp smile.

"Twenty-four hours?" he muttered, looking up at his shiny new ten-year counter. "Good. That's plenty of time to tear this place apart."
