Chapter 1

Ethan ran like his life depended on it. His lungs burned and his legs felt heavy, but he did not stop. The call from the hospital kept ringing in his head. Sophie was sick. Really sick. They said it looked like poison, and she might not make it through the night if they did not operate soon.

City General Hospital rose up in front of him, tall and shiny. Ethan hated how expensive it looked. His family could barely pay the rent on their small apartment. How were they supposed to afford a place like this?

Inside, the air smelled sharp with medicine. Ethan pushed past people until he found the emergency area. Sophie lay on a bed behind a thin curtain. Her face was white as paper. Her chest went up and down too fast, like she could not get enough air.

A young doctor looked up when Ethan came in. “Are you family?”

“I’m her brother. Ethan. What’s wrong with her?”

The doctor’s face was serious. “We think something toxic is in her system. It is spreading fast. She needs surgery right now or her organs will start shutting down. If she falls fully unconscious, we might lose her.”

Ethan’s stomach dropped. “Isn’t there another way? Something less… big?”

“No. Surgery is the only chance.” The doctor was already walking away to another emergency. An ambulance siren wailed outside. The curtain fell shut again.

Ethan stood there staring at his little sister. Sophie was only sixteen. She still had posters on her bedroom wall and homework half-finished on the kitchen table. She was supposed to go back to school next week. Not lie here dying.

Their family had never been rich. Ten years ago their father, Mark, got pushed out of a big company after a fight with the owners. He used to run part of the business. Now he drove a taxi for a small app company and came home tired every night with empty pockets. Customers were few. Bills for electricity, water, and rent never stopped coming. Ethan was twenty. He worked nights singing and playing guitar at a small bar called The Night Owl, plus some shifts waiting tables. It was never enough.

He walked to the front desk on shaky legs.

The nurse behind the counter was blonde and busy with her phone. She was on a video call, eyes wet, voice low and angry. Someone on the other end had just broken up with her. She barely looked at Ethan.

“Patient name?” she asked, still half watching the screen.

“Sophie Rivers.”

She tapped on her tablet. Her eyes widened for a second, then she put on a fake worried face. “You need to put down a ten-thousand-dollar deposit before we can start the surgery. That does not even cover the doctor or the full operation. Looking at the information we have… you cannot pay it.”

Ethan felt the floor tilt. “Ten thousand? By when?”

“Seven o’clock tonight.” Her voice was sharp now. On the phone her ex was still talking. She was trying hard not to cry in front of a stranger.

“This is a big hospital,” Ethan said, trying to keep his voice steady. “Don’t you help families who don’t have money? Even a little discount?”

The nurse’s face went cold. The tears dried up. She stood and pointed a pen at him like a weapon. “This is not a charity. If every poor person came in here begging, how would we get paid? Take your sister to a cheaper clinic if you cannot afford real care. You’re young and healthy. Go sell something. Borrow from someone. Or maybe sell yourself — whatever people like you do. Just get out of my sight before I call security.”

She waved him away like he was dirt. Ethan turned and left, the bright lights buzzing over his head. Outside he leaned against the wall and tried to breathe. Sophie needed that money or she could die. There was no other choice.

He ran to The Night Owl Bar. The owner, Mr. Harris, was wiping glasses when Ethan burst in and told him everything. Mr. Harris listened without a word, then opened the safe under the counter and counted out eight thousand dollars in worn bills.

“Pay me back when you can,” he said. “And don’t do anything stupid.”

Ethan almost cried with relief. Eight thousand. He still needed two more, but he had some savings in the bank. It might be enough.

He headed for the roadside ATM, heart pounding with a little hope for the first time all day. That was when he heard the soft voice.

“The Queen’s Eye.”

An older woman stood in the shadows between two closed shops. She looked Asian, dressed in clothes that seemed from somewhere far away. In her open hand sat a dark stone carved like an eye. It shone deep red under the streetlight.

“This comes from the far East,” she said. Her voice was gentle, almost like music. “It brings good fortune to those who need it most. Ten thousand dollars.”

Ethan knew it was crazy. Sophie was dying. He had just borrowed money for her surgery. But the stone seemed to pull at him. The woman’s words wrapped around his thoughts like warm fog. Before he could stop himself, he handed over the eight thousand from Mr. Harris and the last of his savings.

“I’ll get the rest from the machine,” he mumbled.

When he gave her the final two thousand, the woman smiled. “May the stars bless you.” Then she was gone, like smoke. Only a thin white mist hung in the air for a second.

Ethan stood alone holding the stone. Up close it looked ordinary. Just a carved rock, maybe painted to look old. Rage and panic hit him at the same time.

“What did I just do?” His voice cracked. “Sophie needs that money. She’s dying and I just… threw it away on a stupid rock. I’m the worst brother ever.”

He was about to throw it into the street when a calm voice spoke inside his head, clear and close like someone standing right beside him.

[Greetings. The Queen’s Eye System has been activated. You are at level zero. A task will be given soon. Prepare for what comes next.]

The stone vanished. A small tattoo shaped like a closed eye appeared on the back of Ethan’s hand, glowing faintly red before settling into his skin. The voice felt real. Too real.

Ethan stared at the mark, heart slamming so hard it hurt. Somewhere across the city the clock was ticking toward seven. Sophie still needed that deposit. And now he had nothing left but a strange mark on his skin and a voice that should not exist.

He clenched his fist until his nails dug in, fear and anger and a tiny spark of something new fighting inside him.
