Operating on an Inquisitor
last update2026-11-02 11:07:04

High Inquisitor Vane collapsed onto his hands and knees in the mud, his breath ragged and whistling through his teeth.

The brass optical lens embedded in his left eye socket clicked frantically, spinning back and forth like a broken watch before emitting a sharp pop. A thin ribbon of foul-smelling, scorched black smoke curled out from the copper rim, accompanied by a droplet of dark, fluid blood that trickled down his pale cheek.

[ ACUTE CEREBRAL PRESSURE: CRITICAL ]

The two Executioner Specialists scrambled backward across the wet cobblestones, staring in horror at their twitching commander. They reached for their sidearms, but the sharp, metallic shhhk of steel drew their eyes upward.

Lieutenant Harrow and Lieutenant Kael stood over them, their heavy halberds leveled directly at the Executioners' throats. Behind them, Officer Lyra Vance held her rapier at a low, lethal guard, its blue mana-aura reflecting off the rain-slick stones.

"Touch those holsters," Lyra warned, her voice flat and completely devoid of mercy, "and you won't live to see your commander bleed out."

Vane gritted his teeth, his right eye bloodshot and wide with agony as he looked up at Caelen Voss. The paralysis had spread down his neck, locking his left arm in a rigid, unnatural spasm against his chest.

"You... rogue trash..." Vane rasped, his voice trembling under the sheer weight of the pain. "If I die... the High Council... will turn this entire sector to ash..."

"If you die, Inquisitor," Caelen said calmly, crouching down so his dark eyes were level with Vane's, "you won't be around to care what the High Council does."

Caelen reached into his coat satchel, pulling out a small leather roll of specialized tools: two silver vascular probes, a pair of micro-forceps, and a sealed glass vial of clear, chilled local anesthetic.

"Your mechanical graft was installed by a Guild surgeon who cared more about mounting the brass housing than respecting the dura mater of your brain," Caelen explained, his voice entirely clinical, as if lecturing a class of first-year medical students. "The corrosion has created an isolated pocket of infected fluid right against your temporal lobe. In eight minutes, the pressure will trigger a fatal seizure."

Vane’s breathing turned into shallow, frantic gasps. His right hand reached out, grabbing Caelen’s coat sleeve with a trembling, desperate grip. The arrogant Inquisitor who had stepped into the plaza to execute a sector was now looking at Caelen with the raw, instinctual terror of a dying animal.

"Save... me..." Vane choked out, his pride completely shattered. "Fix... it..."

"Surgery on an Imperial Executioner isn't free, Vane," Caelen replied coldly, not flinching from the man's grip. "First, you order your men to stand down. Second, you revoke Protocol Zero-Hour for Sector 9. And third... you tell me who in the High Council authorized the Poison Guild's bio-refinery."

Vane’s single working eye darted frantically toward his two Specialists, then back to Caelen's dark, unyielding face.

"I... order it..." Vane gasped out, his voice cracking. "Stand down... all units... stand down..."

The two Executioner Specialists hesitated for a fraction of a second before immediately opening their coats and dropping their sidearms onto the wet mud.

"Protocol Zero-Hour... suspended..." Vane wheezed, sweat pouring into his eyebrows. "The authorization... came from Grand Chancellor Sterling... Valerius's father... he controls the central distribution..."

[ SYSTEM MATCH: GRAND CHANCELLOR ARCHIBALD STERLING ]

Caelen’s eyes narrowed slightly. Archibald Sterling. The head of the Supreme Medical Guild and the patriarch of the Time-Bank. The conspiracy didn't stop at the academy or the local refineries; it reached the very apex of the empire's power structure.

"Tim! Bring the folding gurney and the arc-lamp out to the plaza!" Caelen commanded, standing up and unbuckling his leather field kit.

"Doctor!" Tim yelled from the doorway, scrambling to carry the heavy folding table into the rain. "You're operating out here in the open?!"

"The pressure is too high to move him inside," Caelen said, snapping sterile rubber gloves onto his hands with a loud snap. "Every second spent moving him is a second closer to a cerebral rupture. Lyra, hold his head rigid. Harrow, shield the workspace with a oilskin tarp."

"Got it," Harrow grunted, stepping forward with a heavy waterproof canvas sheet, holding it stretched out over the operating table to keep the falling rain off the workspace.

Within thirty seconds, Vane was laid flat on the table under the canvas tarp. The bright, concentrated beam of the mana-arc lamp illuminated the side of his pale, twitching skull.

The world around Caelen slowed to a absolute crawl. The sound of the rain against the canvas overhead became a low, distant hum. In his vision, the skin, muscle, and bone of Vane's left temple dissolved into a 3D translucent anatomical map, highlighting the corroded brass housing, the inflamed nerve pathways, and the dark purple abscess pulsing beneath the skull.

"Anesthetic," Caelen ordered flatly.

He drove the fine needle of the local anesthetic into three precise nerve points around Vane's temporal bone. Vane’s violent spasms instantly subsided as the blocking agent took effect.

"Scalpel."

Caelen held the fine steel blade at a fifteen-degree angle. With a single, smooth, lightning-fast stroke, he made a curved two-inch incision along the hairline just above the brass ocular housing. Blood welled up, but before it could obscure his view, Caelen used a sterile cotton swab to clear the field.

"Forceps."

Caelen inserted the micro-forceps beneath the corroded rim of the brass graft. The metal was locked tight against the bone, crusted with oxidized copper and dried, calcified tissue.

"Hold his jaw, Lyra," Caelen warned softly. "This part requires absolute stillness."

Lyra clamped her gloved hands onto Vane’s jawline, her grip like iron, her eyes locked on Caelen’s steady fingers.

Caelen didn't force the metal out. Using his kinetic vision to track the micro-vibrations of the bone, he applied a precise, pulsed pressure to the top flange.

Click.

A tiny bubble of dark, foul-smelling fluid escaped from beneath the brass rim.

Click... POP.

With a delicate, upward flick of his wrist, Caelen dislodged the corroded brass ocular graft completely from the skull socket, lifting it free and dropping it into a metal tray with a sharp clatter.

[ FOREIGN BODY REMOVED ]

[ PRESSURE RELEASED ]

A sharp, violent rush of dark, purplish fluid drained out of the exposed socket into the collection tube Caelen held ready.

Vane pulled a deep, shuddering, painless breath into his lungs. The intense crimson flush in his right eye receded immediately, replaced by a healthy, clear white. His paralyzed left arm relaxed, falling limp against the side of the table.

[ SYSTEMIC COLLAPSE AVERTED ]

[ VANE EXPIRATION RESET: 18 YEARS ]

Caelen rapidly cleaned the socket with a concentrated saline-alkali wash, applying a layer of sterile silver-gauze over the incision before wrapping a tight linen bandage around Vane’s head.

He pulled off his blood-stained gloves, tossing them into the waste bucket, and checked his pocket watch.

The entire brain procedure had taken exactly three minutes and forty-two seconds.

The two Executioner Specialists stood paralyzed in utter disbelief. They had watched top High Guild surgeons perform hours-long craniotomies inside multi-million-hour surgical suites—yet this rogue professor had just disarmed an explosive brain abscess on a wooden table in the middle of a muddy slum plaza using a hand-held scalpel.

Vane lay on the table, breathing slowly, his single working eye staring up at the canvas tarp overhead. The agony that had been consuming his mind for months was completely gone.

Slowly, Vane sat up. He touched the clean linen bandage on his head, feeling the absence of that horrible, burning pressure behind his eye.

He looked down at Caelen Voss, who was calmly wiping his scalpels clean with an antiseptic cloth.

Above Vane’s head, his deep crimson counter stabilized at a steady [ 18 Years : 04 Months : 02 Days ].

"You... you spared my life," Vane whispered, his dry voice laced with an emotion he hadn't felt in decades: genuine awe.

"I didn't spare your life, Inquisitor," Caelen corrected without looking up from his tools. "I treated a patient. Now you owe this clinic a debt."

Vane stood up slowly, stepping off the table. He straightened his dark leather coat, looking around the empty Iron Market square, then at the hundreds of slum residents peering cautiously out from their shuttered windows.

"Protocol Zero-Hour is officially canceled," Vane declared, his voice regaining a fraction of its former rasping authority. He turned to his two Specialists. "Gather our weapons. We are returning to the Upper Spire."

"Inquisitor?" one of the Specialists blinked. "What do we report to the Grand Chancellor?"

Vane looked back at Caelen Voss—at the towering, brilliant 230-Year golden counter glowing above the doctor's head, and at the steady, terrifying confidence in his dark eyes.

"Report that Sector 9 is under strict medical quarantine," Vane said softly. "And tell Grand Chancellor Sterling... that the doctor is coming for his hospital."
